To Enroll:
When enrolling in a grade other than Kindergarten, the student should bring the following:
- Two (2) Proofs of residency (rental, lease or purchase agreement or mortgage statement, and utility bill).
- Birth Certificate
- Social Security Card
- Immunization records
- Certified and signed copy of custody order/decree, if applicable
- Any other information the student has from his/her previous school You may wish to review our Graduation Requirements.
Immunizations
Ohio state law requires all students to be immunized in order to attend school. Please bring a copy of the immunization record when you register your child. For more information, please contact the school office at 330-325-7971.
